Overview

The Home Centered Care Institute (HCCI) is a national nonprofit organization, founded in 2014 by Dr. Thomas Cornwell, focused on scaling home-based primary care and bringing it into the healthcare mainstream. In addition to being an authoritative source for clinical and operational best practices for house call programs, HCCI is advancing the field through education, consulting, research, and advocacy. HCCI is changing the way healthcare is delivered to millions of medically complex and chronically ill Americans by leading the transformational scaling of home-based primary care through a far-reaching array of programming accomplished through workforce education and training, research on quality and best practices, advocacy around healthcare payer reform and program development.
